Serious Occurrence Reporting

 


Serious Occurrence Reporting
All licensed child care programs located in the City of Toronto are required to report Serious Occurrences to Toronto Children's Services. In addition, family support programs, summer camps, special needs resourcing and ARC programs who receive funding from the City of Toronto are also required to report serious occurrences.

After regular business hours, weekends and government holidays, please call 416-540-3725

 

Reporting Procedure

Serious occurrences need to be reported within 24 hours of the occurrence by calling the City of Toronto Serious Occurrence Line at 416-397-7359 between the hours of 8:30 AM and 4:30 PM, Monday through Friday. After hours please call 416-540-3725 and a Children's Services Consultant, will ensure that the call is followed up.

A written report must be faxed or mailed within 7 days to the Ministry of Community and Social Services, Ministry of Children and Youth. Serious Occurrence Inquiry Reports should be faxed to the Serious Occurrence Specialist at 416-327-0044. The phone number for the Ministry's Toronto Regional Office is 416-325-0652.
