Program Planning Process

Chronic Disease Prevention is important work. In Canada about two thirds of total deaths are due to cardiovascular disease, cancer, chronic obstructive lung disease and diabetes, placing a major demand on the health care system.

The total cost of illness, disability and death in Canada due to chronic disease is over $80 billion annually. As major chronic diseases in Canada share many common behaviours and conditions, there is an opportunity for an integrated and more coordinated approach to Chronic Disease Prevention.

We at Toronto Public Health in the Chronic Disease Prevention Directorate have demonstrated our commitment to reducing the burden of chronic disease by engaging in a process of integrated planning and delivery of comprehensive chronic disease prevention services.

We've made great strides by:
  1. Developing Toronto Public Health's Integrated Chronic Disease Prevention Strategic Framework.
  2. Undertaking an integrated program planning process.
The program planning process consists of the following sections: The work currently underway is to implement the recommendations from the program planning process to enhance the quality of the chronic disease prevention services provided by Toronto Public Health.

Evaluation

An evaluation of the integrated Chronic Disease Prevention (CDP) program planning process was undertaken. The evaluation findings have provided a comprehensive picture of the orientation process based on the experience of participating CDP staff and managers and their satisfaction with the different components of the orientation process. In addition, the evaluation findings capture the knowledge of the managers about integrated CDP program planning as well as a baseline measure of staffs' knowledge of integrated CDP program planning.
