Toronto Public Health is conducting a telephone health survey to collect data on Toronto residents' health behaviours and attitudes towards important public health issues. The results of the study will help us understand the health status of Toronto residents and inform service and policy decisions.
Toronto Public Health has hired an independent research firm to conduct telephone interviews for the survey. Between October 7th and December 18th, 2011, interviewers from R.A. Malatest & Associates will be calling households in Toronto. They will ask for someone 18 years of age or older to complete the survey. Phone numbers will be selected at random and will include some cell phone numbers. The interview takes approximately 20 minutes and is conducted in English. We hope to complete 1,200 interviews by end of the year.
Toronto Public Health and R.A. Malatest & Associates Ltd. will treat the information collected with the highest standards of confidentiality. Survey responses are kept in a secure manner and all data collection procedures follow the provisions of Ontario's privacy legislation.
Participation in the survey is voluntary. By contributing to the survey, participants will help Toronto Public Health better deliver health services and develop policies that address the needs of Toronto residents.
We recognize that your time is valuable and thank you in advance for your support and participation. If you have any questions, please call Toronto Public Health's Surveillance and Epidemiology Unit at 416-392-7450 or seu@toronto.ca.
