Program Goals
TB Homeless Initiative
Prevention, early identification and effective management of TB to reduce transmission among persons who use/work in shelters and drop-ins. |

TB Corrections Initiative
Prevent, control, and reduce the impact of tuberculosis in the Toronto Corrections population and related communities.
|
|
|
TB STOP Drop-In
Provides drop-in services for the Homeless and Under-housed Population on Tuesdays and Thursdays from 11am - 1pm or by appointment (277 Victoria Street, 1st Floor, Toronto, Ontario M5B 1W2, 416-392-7457).
- Conducts symptom screening
- Collects sputum specimens
- Provides referrals to other health care professionals as appropriate
- Administers Tuberculin Skin Tests (TST)
- Skin Tests are administered on Tuesdays or by appointment only
- Skin Tests administered on Tuesday need to be read on Thursday
* Clients must return to the TB STOP 48 to 72 hours after being tested to have their skin test read *
Case Management
Works with an interdisciplinary team of professionals to manage active TB disease and infection in the homeless and underhoused population
- Ensures clients receive appropriate TB treatment and medical follow-up
- Provides education, counselling, and support to TB clients
- Connects clients with health care and social services and liaises with these providers to co-ordinate care
- Conducts contact follow-up
Directly Observed Therapy (DOT)
Provides on-going, in-person support for clients diagnosed with TB
- Meets with clients 2-5 times a week to ensure treatment is taken appropriately; DOT can be at home, TB STOP, or any other pre-arranged location
- Monitors clients for side effects
- Assists with psycho-social issues
- Accompanies clients to medical appointments as necessary
- Provides referrals as appropriate
Active Case Finding (ACF)
Works collaboratively with specific shelters and drop-ins for the early detection of active TB disease.
- Conducts symptom screenings
- Collects sputum specimens to test for TB at the lab
- Takes and provides referrals as appropriate
Corrections
Works with the Ministry of Community Safety and Correctional Services to manage active TB disease and infection in correctional facilities
- Ensures clients receive appropriate assessment, treatment and medical follow-up
- Co-ordinates TB follow-up upon discharge from correctional facilities
- Consults with correctional and probation and parole staff on TB related issues and concerns, including infection control and staff training
- Provides referrals as appropriate
Education and Outreach
Provides educational materials, presentations and in services to clients, shelters, drop in centres, community groups/organizations, correctional facilities and probation and parole offices
