Application to the program is required annually with a different process for first-time and renewal applications:
First-time applications must include a fully completed application form, fulfill all application requirements (see notes 1-5) and include the appropriate application fee (see note 6) paid in full. An application checklist has also been provided to aid first-time applicants in the completion of your application.
Renewal applications for the first four years after the initial application require only a completed renewal application form (no other documentation or fee is required).
