Program innovations
In partnership with community agencies, Streets to Homes developed new programs to better serve people living on the street. We help them find and keep permanent housing.

New programs:

  • Mobile Multi-Disciplinary Street Outreach Team (M-DOT) (PDF 297 kb) - uses its medical and psychiatric expertise to assist clients with serious mental health and addiction issues

  • Fred Victor Centre's Rapid Access Pilot Project - provides 10 units of transitional housing to stabilize clients before moving into permanent housing.

  • Post Incarceration Housing Program - focuses on securing housing (PDF) for individuals at their release from the criminal justice system.

Plus these support and training initiatives:

  • Centralized follow-up supports - 12 organizations deliver follow-up supports once a client is housed

  • Additional training for outreach workers and follow-up staff - through community safety, mental health and addictions, documentation, harm reduction, urban Aboriginal issues and income supports

  • Employment supports - through intensive pre-employment and vocational supports to prepare clients for re-entry into the labour force

  • Moving service - thorugh helping clients access furniture and get settled into their new apartments

  • Income supports - thorugh fast-tracked intake for clients living outdoors and arranged with Toronto Social Services

  • Volunteers - through assists with community development and recreational activities for clients
