Reporting Forms for Housing Help Services
The Shelter, Support and Housing Administration (SSHA) Division requires all funded service providers to report on specific service and outcome data. This data helps SSHA in its role as a service manager, and provides information for City Council and the provincial government.
There have been some changes in 2012. In addition to regular quarterly reporting, service providers (excluding Housing Help Within Shelters) must now complete and submit two forms: the Project Update Form and the Final Reporting Form. The purpose of the Project Update Form and accompanying Excel attachment is to obtain budget information and identify any changes to funded project(s) in 2012. The Final Reporting Form and accompanying Excel attachment is an account of activities and funding, and service trends. Please review the reporting schedule for the submission of each of these reports.
