The Ministry of Health and Long-Term Care (MOHLTC) created and funded a Long-Term Care – Mental Health (LTC-MH) Framework Steering Committee to develop a model and strategies that would provide enhanced mental health support for residents who live in long-term care homes. Following development of the Framework, the MOHLTC provided funding to develop a How-to-Guide for healthcare partners about how to navigate the system and utilize the Framework effectively.

The Long-Term Care Homes and Services Division was privileged to provide leadership in this project, chairing the committee and finalizing the work. The division is pleased to provide a link to the entire LTC-MH How-to-Guide with the approval and support of the MOHLTC, so that it is available as a resource for all system partners as a resource in quality improvement.
