How to complete the notice
It is vital that the information recorded on this document is legible, correct in content and complete to ensure that notification, when mailed, will be directed properly. Please print clearly. An example indicating the requirements and wording of a Notice of Intention to Appear is displayed below.

Defendant
Always the name of the person or company who is registered with the Ontario Ministry of Transportation as being the owner of the vehicle plate.
Current mailing address
Complete this address field with the address of the vehicle plate owner(s) registered with the Ministry of Transportation. If you have recently moved and have not changed your address with the Ministry, please list the address on the vehicle registration document. See how to change your address.
Telephone
Defendant's daytime phone number, including area code.
Infraction number
The eight (8) digit number appearing on the Parking Infraction Notice or the Notice of Impending Conviction.
Challenge officer evidence
Check off either: Yes or No.
Trial language
You have the right to have your trial heard in either official language.
Interpreter
If you require the services of an interpreter for a language other than French, record the language required. An interpreter will be ordered to assist you on your trial date. The Notice of Trial will also indicate that language on the top of the notice.
Signature
Complete the notice form by signing your name and recording the date signed. Present it to the counter clerk for review and filing.
