	Music Video Sheet (PDF)
	You must submit a Production Information Sheet if your production is one of the following production types:   Feature Film Television Series Movie of the Week (MOW) Any television special (short film, documentary or variety show) If you are seeking to shoot a music video, you must submit a Music Video Sheet. Information sheets are not required for student productions or TV commercials.
	Fax Cover Sheet for SPFX / Pyrotechnics / Firearms & Gunfire / Squibs (PDF)
	SPFX Squibs Only / Letter of Intent Form (PDF)
	This form must be filled out by the pyrotechnician when squibs for bullet hits only are used on film sets in the City of Toronto.
	SPFX Firearms and Blank Gunfire / Letter of Intent Form (PDF)
	This form must be filled out by the gun handler when firearms and blank gunfire are used on film sets in the City of Toronto.
	SPFX Pyrotechnics / Letter of Intent Form (PDF)
	This form must be filled out by the pyrotechnician when using SPFX explosives / pyrotechnics on film sets in the City of Toronto.
	Fire Hydrant Tracking Form (PDF)
	This form must be filled out by the Generator Operator for each hydrant and given to the Location Manager or Assistant Location Manager when grounding to a hydrant for film shoots in the City of Toronto.
	Production List Registry Request Form (PDF)
	Once the Toronto Film & Television Office receives your completed form, your information will be added to our registry and you will begin to receive the TFTO production list on a regular basis.
