You are invited to participate in a survey to help us understand COVID-19 in Toronto.  This study is being led by researchers at the following organizations:

Toronto Public Health, Epidemiology and Data Analytics Unit

Ryerson University, School of Occupational and Environmental Health

The survey will ask you questions about yourself, including your employment, medical risk factors, household size and income, racial and ethnic background, gender and sexual orientation, as well as COVID-19 information sources you use. We are seeking to include approximately 2,350 people in this study.  This study has been approved by Toronto Public Health and Ryerson Research Ethics Board.

Your participation in this study may benefit the COVID-19 response in the City of Toronto. With this information, we will be able to:

  • Respond to specific community needs; and
  • Better plan public health programs and other services and supports.

We don’t anticipate that participating in this study will benefit you directly. We also don’t anticipate any harms associated with participating in this study.

This survey will take approximately 20 minutes to complete. The questions that you will be asked are completely voluntary – you can skip any questions that you don’t feel comfortable answering. You can also stop at any time and withdraw your information. If you withdraw your information at any time before the end of the survey, the information gathered will not be used. However, once the survey is completed, you will not be able to withdraw the information. If you choose not to answer these questions it will not affect your access to services.

Data for this study will be collected through CheckMarket, TPH’s standard data collection tool. Data is transferred to a safe and secure Toronto Public Health site accessed only by the research team, and will be stored there and destroyed after 7 years. Your individual responses will only be seen by Toronto Public Health staff and Ryerson University research partners collecting and analyzing the data, and we will follow all laws related to the protection of your Personal Health Information.

Any data that is shared beyond the research team will be presented as a summary with other people’s data, in a way that it can’t identify you. Data from this study will be used in factsheets, reports, and research papers. Results from this study will be disseminated on the Toronto Public Health website.
