You can get your COVID-19 vaccine certificate no matter where you were vaccinated (for example, at a pharmacy, mass immunization clinic, hospital, or primary care setting) or if you have an active exemption. Click here for more information.
If you live in Toronto and received a dose outside Ontario, please document your vaccine in Toronto Public Health’s online form. Recording out of province or out of country doses in COVaxON is important to ensure subsequent COVID-19 vaccine doses can be given safely and at appropriate time intervals. You can document individual COVID-19 vaccines or a full series on Toronto Public Health’s online form.
Document your out of province or non-Ontario stock vaccine
Service requests will be processed as soon as operationally possible . Please note that some submissions may require follow-up with the client and/or with the location where the vaccinations were received. In these cases, processing may take longer.
Clients are encouraged to bring their Out-of-Province receipts with them when getting vaccinated as proof of eligibility.
