COVID-19: Report Workplace Outbreaks

In accordance with the Toronto Medical Officer of Health’s Letter of Instruction, employers must immediately notify Toronto Public Health when they become aware of two or more people who test positive for COVID-19 within a 14-day period in their workplace.

Use the COVID-19 Workplace Reporting Tool if all three of these criteria apply to your business/organization:

• You have a business or organization in the City of Toronto.
  • This only includes workplaces with a postal code beginning with M** ***. If this is not the case, please access the Provincial Public Health Unit Locator, to see what is required by your local public health department.
• You are a person(s) responsible for a business or organization
• Two or more people connected with your organization have tested positive for COVID-19 within a 14-day period (since January 4, 2021).
