The City of Toronto (“City”) maintains the Sexually Transmitted Infection Medication Ordering form (“STIMO”) for the City of Toronto Public Health (TPH) Unit. STIMO is a web-based service provided by the City to enable Toronto Physicians, Nurse Practitioners, and Hospital Pharmacists to electronically submit orders for a select number of publicly-funded medications prescribed for the treatment of specific reportable sexually transmitted infections. When you click the “Agree” button when entering, and by using STIMO, you are agreeing to comply with and be bound by the following terms of use. Please review the following terms carefully. If you do not agree with these terms, you cannot use or gain access to the STIMO form.
Subject to the terms and conditions of this Agreement, you are granted the right to access, and use STIMO for your professional use. You may only access and use STIMO to submit medication requests you are lawfully permitted to access and use. You may only place orders using STIMO if all of the following apply to you:
You will only enter information in STIMO that you know to be accurate. You will report any errors in or concerns about STIMO to the TPH Sexually Transmitted Infection Program by emailing stidrugorder@toronto.ca or if you do not use e-mail, calling 416-338-2373. You are responsible for safeguarding your login credentials, including your TPH-assigned Unique Health Care Provider Identifier and Program Access Code, before, during, and after your use of STIMO. You are responsible for the security of the device, browser, and network that you are using to access the STIMO website and transmit information. You will provide accurate STIMO login credentials where required.
You warrant that the information or material that you provide to STIMO through your access to or use of STIMO does not infringe the rights of any person or entity. Without limiting the generality of the foregoing you shall not:
Any password or right given to you to use STIMO is not transferable.
The City is not responsible, and will not be liable to you or anyone else, for any damages whatsoever, including any indirect, special incidental or consequential damages, arising out of or in connection with your use, or inability to use, STIMO even if the City has been advised of the possibility of such damages.
The City is providing STIMO on an “as is”, “as available” basis. The City does not make any express or implied warranties, representations or endorsement with respect to STIMO including, but not limited to warranties as to usefulness, completeness, accuracy and reliability. Further, the City does not represent and warrant that STIMO will be available or meet your requirements, that access will be uninterrupted, that there will be no delays, failures or errors or omissions or loss of transmitted information, that no viruses or other contaminating or destructive properties will be transmitted or that no damage will occur to your computer system. You have sole responsibility for adequate protection and backup of your data, hardware, and software and to take reasonable and appropriate precautions against viruses and other contaminating or destructive properties.
The Municipal Freedom of Information and Protection of Privacy Act (MFIPPA) and the Personal Health information Protection Act (PHIPA) apply to all information collected by the City, including Toronto Public Health. The information will only be collected, used and disclosed in accordance with MFIPPA and PHIPA. Please refer to the Toronto Public Health Information Practice Statement for more information.
The text, images, and data on STIMO are protected by the Canadian Copyright Act. The City grants you a personal non-exclusive license to read and print copies of your medication orders and those of others you are, in law, permitted to access. You do not gain any ownership rights to any of the copyrighted information you gain access to through STIMO.
The City may, at its sole discretion, terminate or refuse to permit the use of any services on STIMO by any person, for any reason and without notice. You agree that even if the City does not insist on or enforce an item in this agreement, the City still has the right to insist on and enforce that item.
