Coordinated Access to Housing and Supports (Coordinated Access) is a systems-level approach for addressing homelessness that provides a consistent way to assess, prioritize and connect people experiencing homelessness to City-funded housing and supports. Below is the latest list of Housing Providers, Support Providers, and Supportive Housing Providers that receive referrals via the Coordinated Access process. The list is divided into four categories and arranged alphabetically within each group.
Clients who are connected to Coordinated Access programs will be provided with a consent form where they can specify providers they do not wish to share their information with.
