This form is used to request a review of the screening officer’s decision.
If you have received a City of Toronto Parking Violation Screening Decision Notice, and you want to file a review of the Screening Officers Decision, you have 15 days from the notice date to dispute it.
If you wish to dispute the decision, you must complete a Request For Hearing form online between 1 – 15 days, or you may be able to request an Extension of Time to Request a Hearing review within 30 days of a Screening Officers Decision.
You may also be eligible for more time to dispute a notice if there are extenuating circumstances. Requests can be made securely online.
Notices can also be paid online, in-person or by mail.
You may also attend in person to complete an application within 15 days of the Screening Officer’s decision in-person at 40 Orchard View Blvd. 2nd Floor, Suite 253, Toronto, ON.
This form is used to request a change of the scheduled hearing date. This form must be received by the Administrative Penalty Tribunal at least five business days in advance of the hearing date. A change of hearing date can only be requested once.
