This form is used by the owner of the vehicle, the recipient of the parking violation or an authorized representative to request a review of the screening officer’s decision.

Form 1 – Request For Hearing

Deliver your completed application within 15 days of the Screen Officer’s decision in any of the following three ways:

This form is used to request a change of the scheduled hearing date. This form must be received by the Administrative Penalty Tribunal at least five business days in advance of the Hearing Date.

Deliver this completed application to the Administrative Penalty Tribunal at least five business days in advance of the Hearing Date by email to apt@toronto.ca

This form is used to request a copy of the audio recording of a specific proceeding.

Form 8 – Request for Copy of Recording

Filing Instructions

Please contact apt@toronto.ca for instructions on completing the form.  Once complete, deliver the application and make payment in full at one of the Revenue Services counters.

This form is used to request a copy of the Hearing Officer’s reasons for decision.

Form 9 – Request for Written Reasons

This form is used to ask the Administrative Penalty Tribunal to grant a new hearing date.
