Cancellation Form

Lost Licence Form
If you have lost or not received your licence/photo card, please complete this form and send an e-mail to MLSBusinesslicence@toronto.ca. A duplicate fee may be charged.
Annual Return for Corporation Declaration
This form is required to be submitted to confirm the directors/officers of a corporation.
Annual Return for Corporation with Shares Declaration
This form is required to be submitted to confirm the directors/officers of a corporation for the following categories: Private Parking Enforcement Agency, Holistic Centre, Adult Entertainment Club, Body Rub Parlour, Toronto Taxicab Owner, Taxicab Owner
Certificate of Insurance for General Business Insurance
Named Insured: This must be the same name as the applicant/licence holder. If you are submitting as a corporation/partnership, this must be listed under the named insured section.
Address: The operating address must match what is on your application or business licence. This must include any unit numbers.
Certificate of Insurance for Driving School Instructors/Driving School Operators with Vehicle
Named Insured: This must be the same name as the applicant/licence holder. If you are submitting as a corporation/partnership, this must be listed under the named insured section.
VIN: This must match the vehicle ownership.
Note: OPCF/OEF 6D is a separate document that is provided by your Insurance Company or Broker.
Certificate of Insurance for Refreshment Vehicles/Food Trucks/Ice Cream Trucks
Named Insured: This must be the same name as the applicant/licence holder and vehicle ownership. If you are submitting as a corporation/partnership, this must be listed under the named insured section.
VIN: This must match the vehicle ownership.
Certificate of Insurance for Pedicab Owner
Named Insured: This must be the same name as the applicant/licence holder and vehicle ownership. If you are submitting as a corporation/partnership, this must be listed under the named insured section.
VIN: This must match the vehicle ownership.
Certificate of Insurance for Drive Self Car Rental (Car Rental Business)
Named Insured: This must be the same name as the applicant/licence holder. If you are submitting as a corporation/partnership, this must be listed under the named insured section.
Address: The operating address must match what is on your application or business licence. This must include any unit numbers.
VIN: This section must show all vehicle VIN’s or must mention that the policy covers all vehicles owned and operated by the named insured.
Certificate of Insurance for Taxicabs/Limousines
Named Insured: This must be the same name as the applicant/licence holder. If you are submitting as a corporation/partnership, this must be listed under the named insured section.
VIN: This must match the vehicle ownership.
Certificate of Insurance for Road Allowance Permits
Named Insured: This must be the same name as the applicant/licence holder. If you are submitting as a corporation/partnership, this must be listed under the named insured section.

When submitting a photo for your renewal or application, it must comply with the Licence Application Photo Requirements.
