Adult Day Programs provide a wonderful opportunity for seniors living in the community to have “a day out”, participate in social programming and connect with peers. They are offered in three City of Toronto operated long-term care homes:

The programs offer a variety of quality activities and services, in a safe and supportive environment, to individuals living in the community who are physically frail, have a cognitive impairment or are socially isolated.

Each day, participants have the opportunity to take part in a variety of social, recreational and physical activities, including special events and wellness education. Participants receive nutritious meals and snacks and assistance with activities of daily living.

Service provision is respectful of culture, ethnicity, family traditions, community, language, sexual orientation and spiritual beliefs of the members.

Program Goals

  • Preserve clients’ level of mental and physical functioning
  • Provide respite and support to caregivers
  • Help individuals remain at home in their own community

Requirements

Adult Day Program applicants will be assessed for eligibility to the program. A Day Program agreement must be signed for all new clients prior to the first attendance day.

Applicants for the Day Program must meet the following:

  • Have assessed needs which can be met by the program
  • Have a valid Ontario Health Card number and be an Ontario resident
  • Caregiver assessed and in need for regular periods of respite, which can be met by the program
  • Be self-medicating (staff may assist with reminding them to take their medication at the scheduled time)
  • Able to weight-bear and require no more than one-person assistance with transferring
  • Not active exit-seeking or high-risk behaviours
  • Independent smoker, a minimum of 9 meters from the entrance of the building
  • Must not require heavy physical care or have severe cognitive impairment.

Note: Supportive Housing clients are eligible to attend the program and are subject to the same assessment process, eligibility criteria and fee structure.
