Provincial regulations no longer require proof of vaccination, an active medical exemption or participation in a Health Canada approved COVID-19 vaccine trial to access businesses or settings. Owners and operators of those spaces may choose to continue to require proof of vaccination at their discretion.

Enhanced vaccine certificates with scannable QR code are available for all vaccinated Ontarians to download. You can save the electronic version of your vaccine certificate with a QR code to your phone or print a paper copy.

If you have a green photo health card:

  • You can log in to the provincial portal to download your enhanced vaccine certificate with QR code.
  • To log in and get your certificate, you will need:
    • a green photo health (OHIP) card (you will need numbers from both the front and back of the card, expired cards will be accepted)
    • your date of birth
    • your postal code (the one associated with your health card)

If you have a red and white health card or no health card:

  • Call the Provincial Vaccine Contact Centre at 1-833-943-3900. The call centre agent can email or mail you a copy of your enhanced vaccine certificate with QR code. If the Contact Centre is unable to assist, please request support by contacting Toronto Public Health at 416-338-7600, Monday to Friday, from 8:30 a.m. to 4:30 p.m.

What to do if there is an error on your vaccination receipt:

If there is an error on your vaccination receipt, including your name or date of birth, please contact the organization where you received the vaccine to request a correction.

If you received your vaccine at a City of Toronto immunization clinic

Please call Toronto Public Health at 416-338-7600, Monday – Friday from 8:30 a.m. to 4:30 p.m. You may be asked to leave your contact information so that a representative can call you back to correct the information.

If you received a vaccine outside Ontario that has been documented by Toronto Public Health

Please call Toronto Public Health at 416-338-7600, Monday – Friday from 8:30 a.m. to 4:30 p.m. You may be asked to leave your contact information so that a representative can call you back to correct the information.

If you received your vaccine at a hospital clinic, hospital-run community clinic, or pharmacy:

Please contact the hospital or pharmacy location where you received your vaccine.

If you would like the name on your enhanced vaccination certificate changed to a chosen name that is not your legal name, complete the Chosen Name Survey.

You will be asked to provide:

  • The name currently on your enhanced vaccination certificate
  • Your date of birth
  • Your health card number (or a photo of your government issued ID with the name currently on your enhanced vaccination certificate if you do not have a health card)
  • Two pieces of identification or documentation with your chosen name, which includes a name, birth date, and photo ID. Acceptable forms of documentation are:
    • Driver’s license
    • Birth certificate
    • Ontario photo card
    • Passport
    • Piece of registered mail
    • Pay stub
    • Student card
    • Library card
    • Club or organization ID cards
    • Government issued identification from other jurisdictions including foreign passports, other provincial or territorial health cads, or driver’s license

Some residents may qualify for an exemption from receiving the COVID-19 vaccine for medical reasons. Someone granted a medical exemption may download a certificate with QR code for use under Ontario’s Proof of Vaccination Policy. To request a medical exemption please see your healthcare provider. Only a physician or registered nurse in the extended class (nurse practitioner) may submit a medical exemption request to Toronto Public Health. Submissions directly from members of the public will not be accepted.

If you live in Toronto and received a first or second dose outside Ontario, please document your vaccine in Toronto Public Health’s online form. You can document individual COVID-19 vaccines or a full series in Toronto Public Health’s online form. You will be required to submit proof of vaccination.

For vaccines received in other Canadian provinces and territories, proof of vaccination requires submission of an enhanced vaccination certificate with QR code. Please consult the relevant provincial or territorial website for information about how to obtain the enhanced certificate.

Some people vaccinated in Ontario received vaccine from what is referred to as non-Ontario stock. This refers to vaccine that was not allocated to Ontario or recorded in the provincial vaccine management system. Generally these doses were delivered at institutions such as correctional facilities, military groups or embassies. People who received one or two doses of vaccine from the non-Ontario stock may also document them through Toronto Public Health’s online form.

Booking a Third or Booster Dose Before Dose(s) is Documented

If you are eligible for an additional dose(s), including an mRNA dose needed to complete your series or a booster dose, you can book an appointment at any City-run immunization clinic using your green photo Ontario Health Card (OHIP) number through the Provincial portal or by calling 1-833-943-3900 (TTY 1-866-797-0007). With an older red and white OHIP card number or with not OHIP number at all you can book an appointment by calling 1-833-943-3900 (TTY 1-866-797-0007). Walk-in appointments are also accepted. You do not need to wait until your out-of-province dose(s) is documented by Toronto Public Health.

Wait Times for Processing Out-of-Province Doses

Most service requests will be processed within two weeks of submission. Some submissions may require follow-up with the client and/or with the location where the vaccinations were received. In these cases processing may take up to five weeks.

Clients are encouraged to bring their OOP receipts with them when getting vaccinated as proof of eligibility.
