Provincial regulations no longer require proof of vaccination, an active medical exemption or participation in a Health Canada approved COVID-19 vaccine trial to access businesses or settings. Businesses and organizations may voluntarily choose to check for proof of vaccination.

What to do if there is an error on your vaccination receipt:

If there is an error on your vaccination receipt, including your name or date of birth, please contact the organization where you received the vaccine to request a correction.

If you received your vaccine at a TPH vaccination clinic

Please call Toronto Public Health at 416-338-7600, Monday – Friday from 8:30 a.m. to 4:30 p.m. You may be asked to leave your contact information so that a representative can call you back to correct the information.

If you received a vaccine outside Ontario that has been documented by Toronto Public Health

Please call Toronto Public Health at 416-338-7600, Monday – Friday from 8:30 a.m. to 4:30 p.m. You may be asked to leave your contact information so that a representative can call you back to correct the information.

If you received your vaccine at a hospital clinic, hospital-run community clinic, or pharmacy:

Please contact the hospital or pharmacy location where you received your vaccine.

If you would like the name on your enhanced vaccination certificate changed to a chosen name that is not your legal name, complete the Chosen Name Survey.

You will be asked to provide:

  • The name currently on your enhanced vaccination certificate
  • Your date of birth
  • Your health card number (or a photo of your government issued ID with the name currently on your enhanced vaccination certificate if you do not have a health card)
  • Two pieces of identification or documentation with your chosen name, which includes a name, birth date, and photo ID. Acceptable forms of documentation are:
    • Driver’s license
    • Birth certificate
    • Ontario photo card
    • Passport
    • Piece of registered mail
    • Pay stub
    • Student card
    • Library card
    • Club or organization ID cards
    • Government issued identification from other jurisdictions including foreign passports, other provincial or territorial health cads, or driver’s license

If your personal circumstances require that you have a medical exemption (such as for work or travel), please follow up with your health care provider.

If you live in Toronto and received a dose outside Ontario, please document your vaccine in Toronto Public Health’s online form. You can document individual COVID-19 vaccines or a full series in Toronto Public Health’s online form. You will be required to submit proof of vaccination.

For vaccines received in other Canadian provinces and territories, proof of vaccination requires submission of an enhanced vaccination certificate with QR code. Please consult the relevant provincial or territorial website for information about how to obtain the enhanced certificate.

Some people vaccinated in Ontario received vaccine from what is referred to as non-Ontario stock. This refers to vaccine that was not allocated to Ontario or recorded in the provincial vaccine management system. Generally, these doses were delivered at institutions such as correctional facilities, military groups or embassies. People who received one or two doses of vaccine from the non-Ontario stock may also document them through Toronto Public Health’s online form.

Booking a Dose Before Previous Dose(s) are Documented

If you are eligible for your next dose(s), you can book an appointment at participating pharmacies and select doctors’ offices. You do not need to wait until your out-of-province dose(s) is documented by Toronto Public Health to receive your next dose. We recommend you call in advance to confirm.

Wait Times for Processing Out-of-Province Doses

Most service requests will be processed within two to four weeks of submission. Some submissions may require follow-up with the client and/or with the location where the vaccinations were received. In these cases, processing may take longer.

Clients are encouraged to bring their Out-of-Province receipts with them when getting vaccinated as proof of eligibility.
