The City of Toronto is testing a new COVID-19 online screening form at six community recreation centres. The online assessment can be completed in advance, at any time on the day of your visit. Bring a screen capture or copy of your confirmation with you to show staff on site to enter the facility. If you cannot complete the online screening, staff are available to help you complete an in-person screening on site.
