Requests for trial can be made online at toronto.ca/CourtCaseLookUp by completing a request for trial online transaction.

Requests for trial can also be made by completing a Notice of Intention to Appear (NIA) form and sending the completed NIA by mail to the address located on the back of your Offence Notice.

Use Schedule A if you are applying for an extension of time to pay on more than one parking ticket.  If you need more space to state your reasons for extending the time to pay, you may also complete the appendix form.

By providing credible and trustworthy information, you may sign the reopening application form without the need to have an affidavit commissioned.

To request an interpreter, you can fill out an Interpreter Request form and return it by mail to the appropriate Toronto POA Court Office (where the certificate/information is filed).  If a court date has been scheduled, you must complete and file your request four weeks before your scheduled court date.

To request disclosure, complete this form and deliver it by mail to the prosecutor’s office at the court location where your trial will be held.
