Toronto Long-Term Care Homes and Services offers Supportive Housing in various locations across Toronto. We provide supportive services, such as assistance with personal care, light housekeeping and laundry, medication reminders, security checks and light meal preparation to eligible clients who are tenants in designated buildings contracted under our Supportive Housing Program.
In addition to the supportive services listed above, our commitment to wellness and health promotion is a hallmark of our program. The on-site Registered Practical Nurse (RPN) at each of our locations offers health promotion activities and wellness education to assist clients in maintaining their independence. Our care and service is flexible to meet clients' changing needs. The level of support provided is agreed upon by the client, according to their abilities and needs.
Individuals living in supportive housing do not require the level or intensity of care available in long-term care homes, but benefit from to intermittent 24-hour on-site support, including night time safety checks, as warranted. We promote independence, client autonomy and aging in place within a stable, safe, housing environment.
How the program works
Under the Supportive Housing Program, individuals pay rental charges directly to the landlord. There is no direct cost to the client for the supportive services. Instead, the personal support and homemaking costs for eligible clients are paid by Toronto Long-Term Care Homes and Services, through funding provided by the Ministry of Health and Long-Term Care and the Local Health Integration Networks (LHINs).
Where is supportive housing available?
Our supportive housing sites are located across Toronto, in designated buildings. The on-site RPN at each site can provide details about the building, tenancy requirements and the application process.
Getting started - the assessment
The on-site RPN will complete an assessment to determine if your support needs can be met through the program. Once admitted to the program, the RPN will visit you regularly. Our program serves individuals 59 years of age and over who are in need of the services and are medically stable.
