You can request access to general records or personal information (information about yourself), or request a correction to personal information using the Access/Correction Request form (PDF)

You will need to have the latest version of the FREE Acrobat Reader on your computer to read this file.

Completed forms should be sent to:

You can also fill out the form at one of the civic centres.

Please include a cheque or money order for the required fee of $5.00. Do not mail cash.

Unless otherwise indicated all fees are payable to the City of Toronto.

Fees
required for all freedom of information requests
Application fee $5 Must accompany your request. The fee is mandatory and cannot be waived.
Fire incident reports $35 Must accompany your request. Includes application fee and cost of removing confidential information.
Appeal fee - general records $25 Payable to the Information and Privacy Commissioner.
Appeal fee - personal information $10 Payable to the Information and Privacy Commissioner.
Appeal fee - personal health information no fee  
Photocopies $0.20 per page  
Computer programming $15 per ¼ hour If needed to develop a program to retrieve information.
Floppy disks $10 per disk  
Search time $7.50 per ¼ hour Required to search and retrieve records.
Record preparation $7.50 per ¼ hour Required to prepare records for release (removal of confidential information).

What happens when you submit a form?

Building Plans
The Building Division and City Clerk's office have developed a policy and procedure (PDF) for the routine disclosure of building plans in accordance with the Municipal Freedom of Information and Protection of Privacy Act (MFIPPA). The policy came into affect December 5, 2005.

You will need to have the latest version of the FREE Acrobat Reader on your computer to read the policy and procedure.

Requests sent in error to other government agencies
The City responds to freedom of information requests on behalf of City divisions only. If your request is for records from another government agency, you should make your request to the other agency directly.

Here is a list of institutions the City of Toronto often receives Freedom of Information (FOI) requests for in error. These institutions have their own FOI offices that process requests made under MFIPPA for records that are in their possession:
