Toronto Paramedic Services provides a wide range of information through its service teams in Operations, Operational Support, Communications Centre, and Program Development and Service Quality. 

The types of records routinely available from Toronto Paramedic Services are as follows:

  • Ambulance Call Report 
  • Incident Summary Report 
  • Audio recordings of incoming 911 call(s) to Toronto Paramedic Services 
  • Statutory Declaration of Birth

How to Request Records

Patient/Family/Power of Attorney Request

  1. Mail or email a completed Authorization to Disclose Personal Information Form
  2. Mail or email a copy of a piece of government photo identification
  3. Mail a cheque for the required amount made out to “Treasurer, City of Toronto”
  4. Requests from individuals other than the patient must include proof of Power of Attorney for Personal Care or other legal authority

A copy of the requested documents will be sent by registered mail to the mailing address provided on the form.

Please allow 30 days for processing from the time the form and payment have been received.

Note: For disclosure requests pertaining to the personal health information of deceased patients, consent must be received from the estate trustee or the individual who has assumed responsibility for the administration of the deceased’s estate, if the estate doesn’t have a trustee. For these requests, proof of the requestor’s authority to consent to the disclosure of the records must be submitted (e.g. copy of the Last Will and Testament or Certificate of Appointment of Estate Trustee without a Will.)

Legal Requests

  1. Mail or email the request on company letterhead.  Please include the following details:
    • Patient’s name
    • Patient’s Date of Birth
    • Date, time and location of the incident
    • The hospital the patient was transported to (if applicable)
  1. Mail or email a signed patient authorization form
  2. Mail a cheque for the required amount made out to “Treasurer, City of Toronto.” For updated fees, see Routine Disclosure Plan
