The Wellness Report is a checklist-based form that compiles on-site activities, including testing, cleaning and inspecting, with supporting documentation to verify that constructed water and sewage infrastructure is operational and constructed to City standards and is in satisfactory condition with all deficiencies addressed.

The report is required to fulfill regulatory requirements for the City’s Municipal Drinking Water Licence and Consolidated Linear Infrastructure Environmental Compliance Approvals. It is used only for municipal infrastructure and it does not apply to assets governed by private Environmental Compliance Approvals for privately owned, operated and maintained systems.

The Wellness Report must be completed and submitted at two stages:

  1. Acceptance: Required to start the warranty period, these items are required post-construction when the infrastructure is put into service.
  2. Assumption: Required at the end of the warranty period (typically two years), these items are required for the City to assume ownership of the infrastructure.

The report includes roles and responsibilities of the external consultant (working on behalf of developers or third parties), City inspector and City case manager, with some tasks to be completed during construction and other tasks after the infrastructure is operational.

The external consultant is responsible for managing the on-site construction and notifying the City inspector in advance so they can ensure they are on-site for the required inspections outlined in the Wellness Report.

Note: The Wellness Report captures minimum requirements. For complex infrastructure such as trunk sewers, transmission watermains, drop structures, or non-linear infrastructure (such as pumping stations), there may be additional requirements.

Effective March 1, 2026, the new version of the Wellness Report must be used for all infrastructure constructed by developers and third parties.

  • For infrastructure where acceptance was issued prior to March 1, 2026, the May 2023 or March 2026 version of the Wellness Report can be used.
  • Projects governed by overarching agreements that specify an earlier version of the Wellness Report may continue using that version. However, using the latest version is recommended, as it aligns with current City requirements and clearly outlines what is required and when.

The external consultant is responsible for completing and submitting one digital PDF of the completed Wellness Report, including required attachments and appropriate City inspector sign-offs to the City case manager. All questions should be directed to the City case manager.

The City case manager is responsible for verifying the completeness of the Wellness Report before signing off and submitting the completed report with the require attachments to TWwellnessreport@toronto.ca.
