Routine disclosure fees are changing April 1, 2026 from $104.00 to $106.00.

Application Process

  • The application fee is $104 (effective April 1, 2025). Please note that this fee is HST exempt.
  • The fee is in accordance with Toronto Municipal Code Chapter 441, Fees and Charges.
  • A copy of the incident report will be sent via secure file transfer to the email address you provide.
  • Please allow a minimum of 30 business days for processing from the date your form and payment are received.

Submitting Your Request

  • Complete the online form and provide payment by credit card.
  • Or, print the form and mail it along with cheque or money order.
