Use this form to request detailed information about an
incident where Toronto Fire Services responded.

Application Fee

The application fee is $100 (effective April 1, 2024). Please note that this fee is HST exempt.

• Complete online form and provide payment by credit card.

• The form can be printed and mailed to our office along with your cheque or money order.

A copy of the incident report will be sent by secure file transfer to the email address provided.

Please allow 30 days for processing from the time the form and payment have been received.
