Organizations that were funded through the 2009 (2009/2010) Community Service Partnerships (CSP) program are required to submit a year-end program report after the end of organization's funding year (same as the organization's fiscal year).

Please follow the instructions provided in the on-line application system and fill out your 2009 CSP year-end report on-line.

Due date and contact
To find the due date for your 2009 YER report and your Agency Review Officer, please look at the list YER due date for 2009 CSP recipients (PDF).

To request a deadline extension, please also fill out and fax in the Request for year-end report extension (Microsoft Word).

If you have questions or want to discuss your reasons for an extension, please contact your Agency Review Officer (PDF).

Community Funding Programs Unit has sent an announcement e-mail titled "2009 CSP year-end report information" from cgis@toronto.ca to your organization's CSP contact persons of the 2009 CSP funding recipients in early April 2010.
